The real answer is simpler and more uncomfortable than most of us were taught.
It starts with anatomy. Not textbook anatomy. Real anatomy. Ultrasound anatomy.
For years, we injected into “deep fat compartments” as if they were closed, well-defined spaces.
They’re not.
In our new clinical study, using real-time dynamic ultrasound in 100 untreated facial sides, we found something very different:
There are no closed compartments.
There is one continuous deep fat layer three-dimensional, interconnected, structured by fascial blades and fibrous septae extending from the temple across the midface.
This changes everything.
Filler doesn’t just stay where you inject it.
It moves. It follows this anatomical network no matter which instrument you use.
We visualized direct connections between the temple, the lateral midface, and the medial midface.
And we identified a key structure most injectors never think about:
The deep preauricular fat.
A deep reservoir where filler can accumulate especially after lateral cheek treatments.
We didn’t stop at ultrasound.
We confirmed these findings with cadaver studies and histology.
This is not theory.
This is real-time, dynamic ultrasound anatomy.
