We report a challenging case of a tympanic paraganglioma extending to the protympanum and Eustachian tube, with carotid canal erosion and anterior canal wall proptosis.

The lesion was completely resected via a fully transmeatal endoscopic approach, including canaloplasty with total tympano-meatal flap elevation and repositioning. This technique provided excellent visualization, precise hemostasis, and preservation of critical structures, underscoring the safety and efficacy of endoscopic ear surgery.
