The causes of acne can be explained by The Acne Triangle.

The bacteria linked to acne is known as Propionibacterium acnes. It resides in the sebaceous glands of the skin and causes acne when its numbers rapidly increases in the sebum, causing inflammation.

Poor skin exfoliation, resulting in excessive dead skin cells, causes pores (the opening of the hair follicles) to become blocked and form comedones.

Sebum is an oily, waxy substance produced by the sebaceous glands in the skin and secreted through the hair follicle. Sebaceous glands are present all over the skin of the body but are more abundant in the face and scalp. The function of sebum is to protect, lubricate and moisturise the skin. Increased sebum production, caused by increased androgen (hormone) levels that normally occurs during puberty, is the main contributor to acne. Excessive sebum blocks the pores and causes inflammation. In addition, sebum aids the growth of P.acnes.

Acne treatment targets all three aspects of The Acne Triangle. Reducing sebum production is the most important goal in the treatment of acne, followed by unblocking clogged pores.

Understanding acne is the first step to treating it. Having a basic knowledge and understanding of the structure of the skin and how it functions is key to managing many skin conditions such as acne.

The medical name for acne is Acne vulgaris. It is a chronic inflammatory skin condition, which means it is a long term condition that causes inflammation of the skin. It is characterised by blockage and inflammation of the pilosebaceous unit (this is the hair follicle, hair shaft and sebaceous gland shown in the cross-section of the skin diagram). It presents as lesions which can be non-inflammatory (comedones), inflammatory (papules, pustules and nodules) or a mixture of both. I will be going into more depth about these acne lesions in a future post.

Complications of acne include skin changes such as scarring, post-inflammatory hyperpigmentation (PIH) or depigmentation and psychosocial problems such as depression and anxiety.

In skin of colour, acne is particularly challenging because of the potential for complications including PIH and keloid development, which skin of colour is more predisposed to.
