A sudden episode of complete eyelid closure led to enormous stress for our patient.

Blepharoplasty is a surgery for improving the aesthetics of the eyelids, and consequently of the face.

The lower eyelid were treated using a hidden or scarless approach, and a subtle titrated fat transfer was done for lower eyelid.

The upper eyelid was challenging too with loss of brow hair, and a tattooed brow, and a wide brow-lid height.

Her upper eyelid incision stays well hidden in the eyelid fold.

Congenital Entropion (Inturned eyelid) of the Lower Eyelid for this child, led to continued redness, watering and worst: a persistent large corneal epithelial defect.

She was operated recently, following which she returned back to her town, and we continue follow up with photographs.

These are before and first post surgery day photos of the child. This condition is not common in Maharashtra, unlike in North-eastern Indian.

During the six years that I operated in Meghalaya, I had the privilege of operating numerous children for this condition. I have tried a variety of surgeries.

But this technique of surgery that I have used here in this case has been the best one and my favourite by far. Traditional Hotz and Jones both often lead to either excess skin excision or significant cosmetically disabling eyelid retraction.

This above method allows minimal skin and Orbicularis excision with a subtle lower eyelid retractor tightening, using box sutures that lead to magical torsion of the eyelid, without needing deeper bites. Works like a charm, and does not make the eyelids look too asymmetrical. Thanks to my dear friend Dr Stephanie Young who shared what she learned with Prof Yoon-Duck Kim. A game changer in my work on such kids in Meghalaya; and the rare one in Maharashtra.

Congenital severe ptosis surgical correction, 2 months post-surgery results.

In such severe ptosis as above, wherein levator based surgery was chosen, it is important to aim for a little overcorrection, as all these eyelids tend to droop down a tad in the initial 6-12 months after surgery.

This patient will do well long term based on his 2 months after surgery results.

Eyelid lesions as these can be removed to aesthetic results, that restore natural anatomy and contour of the eyelid, by means of meticulous and careful surgery.
