My super strong sister has knee pain!ā€¼ļøšŸ‘Æ

Fortunately for her, I’m a physical therapist šŸ’ŖšŸ¼

This is a very condensed version of what a first visit looks like when I see a new patient. But with the limited time we had, we were able to make some good change!

Subjective: knee pain (mostly at patellar tendon) during terminal knee extension, hamstring stretching, and post-run

Objective: tender at patellar tendon, underutilized quads, hip external rotators, and abductors, and decreased single leg strength on LLE

Assessment: left patellar tendinopathy

Plan: load management & working in rest days, add sufficient run warm up to activate weaker muscles, gradually load patellar tendon so it’s better able to withstand the demand!

Recently I did a poll asking followers which muscle (or muscle group) is the most undertrained in rehab. The response was overWHELMINGLY: adductors!!

With that in mind, I thought what a fun opportunity to show a few ways to train these bad boys!

Adductors are key stabilizers at the hip during single leg stance, they help control femoral rotation through gait, and they co-contract with your glutes to keep the pelvis level when you’re loading one leg. They also ADDUCT the hip - shocking, I know

Most people (myself included) forget to utilize this muscle group in training. Why? I don’t know. They’re a chunky group of muscles that plays a big part in back, hip, knee, and even ankle rehab.

Note: I’ve included a variety of movements here for varying levels of strength / fitness. The Copenhagen with a diagonal reach is absolutely diabolical just so you know

Here are some of my fave shoulder exercises right now! Because I’ve noticed a tendency to favor my right side and constantly be internally rotated and flexed on that right shoulder while holding my baby, I’m trying to find things that feel good and work the rotator cuff / scap stabilizers in different positions (especially overhead control)

Overhead shoulder stability requires more than glenohumeral control, the scapula must upwardly rotate as the thoracic spine extends and rotates beneath it, driven by the serratus anterior, upper trapezius, and lower trapezius force couple.

One of the beauties of starting a PT clinic, being a mom, and creating content is that every day is a bit different! Being a clinician scratches the itch of problem solving for me, being a mom fills my cup emotionally, and the content side of things helps push my creative brain more 🧠

As a physical therapist, my philosophy is this: mobility, stability, then motor control.

If there’s limited range at a joint (or multiple joints), trying to strengthen around the partial range could be helpful but might not yield the best results. First I like to address mobility deficits through mobility under load, sometimes a static stretch, dynamic movement, etc.

Once we’ve established better range, I like to think of stabilizing in and around that joint as the next plan of action. If we’re stable, we can move into phase 3: motor control

Now, with a good baseline mobility and stability level, we can challenge this joint through varying ranges of motion and resistance levels to improve overall strength and capacity of the tissues! YAY!

Did all of this happen in one session like magic? No. True change takes T I M E.

But I wanted to trial a few of these principles to see how he felt afterwards. Good news is he’s feeling good + I know we are on the right track!
