Trying to figure out how to optimize my clinic schedule and would love to hear feedback in case I’m missing something!!! 
 
Personally though, I think starting morning and afternoon clinic with a 15 minute slot is not ideal because: 
- MA’s clock in at 8:00 am/1:00 pm and rooming takes maybe 5 min. Assuming your pts show up on time, it is still feasible to complete the appointment on time but may suck. 
- But! If your pt shows up 14 min late (bc they have a 15 min grace period), you still have to see them. And this will inevitably and perpetually make you late every day. This means longer wait times for your other patients, you feeling stressed out → burning out. 
- I currently have 8:00-8:15, 11:45-12:00, 1:00-1:15, 4:30-4:45 appointment slots, but I’m hoping that by making these time slots templated for 30 min, it can help my clinic run more smoothly. 
- If you average about 15-20 min per pt or less, then these 30 min blocks can give you an opportunity to catch up and close encounters more efficiently. Making all your other time slots 15 min instead of alternating 15/30’s can also provide your patients with more access, but the downside is that you will likely have more volume than before. 
 
I don’t think there’s a “perfect” template, but I definitely think there is a way to optimize for efficiency based on your own clinic stats. What has worked well for you? Let’s help each other out!

Thyroid labs can be confusing, but understanding just two numbers (TSH and FT4) can help you make sense of what’s really going on. 
 
Patients often see the TSH flagged on screening and think that low TSH means that their thyroid hormone is low. 
 
However, your HPT axis functions as a negative feedback loop. So when you have too little thyroid hormone, your TSH actually increases to stimulate more thyroid production (low FT4, high TSH). 
 
There are times when your thyroid hormone is normal, but your brain senses that it’s not. Sometimes, this can happen in early thyroid dysfunction, but it doesn’t always mean we need to treat! A lot of it depends on your symptoms, TSH levels, your existing conditions (cardiac, pregnancy, etc), and if you have antibodies present. 
 
Because thyroid labs can be affected by recent illness, medications, supplements, etc, it is important that we REPEAT and TREND thyroid labs before starting treatment!

Is berberine worth your money? 
 
👍 Benefits: improves lipid profile and blood sugar 

👎 Risks: quality of studies are mixed, GI upset, does not decrease risk of cardiovascular events, not safe in pregnancy 

💊 Dosing: variable but typically 1000-1500 mg in two or three divided doses/day 

🤔 My take: worth a try for people with mildly high cholesterol who do not meet indication for prescription medication and have low cardiovascular risk, people with increased risk of insulin resistance

Fibromyalgia (FM) is complex, but non-restorative sleep is a consistent feature and driver of pain for FM patients. 
 
The thought behind this medication is that if we restore normal sleep architecture, particularly the deeper, restorative stages of sleep, then we can help improve FM pain. 
 
The sublingual formulation of cyclobenzaprine (Tonmya, FDA approved 8/2025) was designed to bypass first pass liver metabolism to work faster at a lower dose while limiting metabolites that may lead to next-day grogginess. 
 
But the price can be steep for Tonmya compared to generic oral cyclobenzaprine, so is the potential pain reduction worth the money?

When patients come in skeptical, I show them this video.  
 
👃 I get that squirting water up your nose feels terrible. No one likes it. 
 
👃 But when you are congested, you need large volume saline like this to help flush out stubborn mucus, allergens, or debris that has clogged your sinuses.
