Here’s a procedure you’ve probably never heard of: an alarplasty.

What is this and why would someone ask me to do this?

The width of the base of the nose is something that varies significantly among ethnicities, but for some, they feel their nose is too wide for their face.

To address this, I perform what is called an alarplasty, where we make the incisions shown in the carousel. This allows me to narrow the base of the nose to reduce the apparent width.

This procedure was performed under mild sedation in the office, and she returned 1 week later to have her sutures taken out.

Because her nose is already pretty, she didn’t need much else. I most often use this maneuver during a rhinoplasty to narrow the base of the nose while performing the more commonly requested changes in other parts of the nose.

She is only 1 month out in these photos, but she already looks incredible and her scarring already is completely undetectable.

Energy based devices irreversibly change the tissues of your face and neck.

I’ve operated on faces that have never had any previous cosmetic treatments and I’ve operated on patients that have had extensive nonsurgical treatments before my facelift, and I can tell you from experience that they are two completely different scenarios.

I’m not here to fear monger. But I will tell you that those devices absolutely change the tissues of your face and neck, and can make it very difficult to get a predictable result after a facelift.

By scarring down the soft tissues under the mantra of “skin tightening,” it effectively is turning the soft, pliable layers of your neck into a block of concrete. My patient yesterday had her platysma muscle completely fused to her neck skin with zero subcutaneous fat. This is not a good thing.

What does this result in? More swelling after surgery because the lymphatic vessels are gone. More soft tissue trauma during surgery because I have to dissect out tissue layers that are fused together. A longer recovery and potentially a compromised result because of all these things.

If a facelift is in your future, be very careful how you treat your face and neck!

Until next year, AAFPRS!!

I got to spend last weekend in Las Vegas at the AAFPRS Annual Meeting! It was a great opportunity to step away from the day-to-day and spend time learning more about the field I’ve worked hard to build a career in.

I’m especially proud to have been invited to speak and share my expertise, while also having the opportunity to learn from so many talented colleagues across the industry. Seeing old friends, connecting with new ones, and exchanging ideas is one of the most valuable things about this community.

I’m incredibly grateful to my colleagues and the AAFPRS for putting on a fantastic meeting and helping me get beautiful results for my patients.

Your brows and lids need to work together.

Sometimes, addressing upper eyelid hooding is as simple as an upper blepharoplasty, where I remove the excess skin that's weighing it down. However, if the brow is also low and approaching the eye, treating only one of these areas won’t look right because we’re not addressing both problems.

A brow lift does not make you look surprised, because it is not purely a vertical lift. While the procedure can include vertically lifting the brows, my primary focus is often on the tail of the brows to take weight off the outside of the eyes.

When you perform both procedures together, an upper blepharoplasty and a brow lift, it can be a great way to restore harmony, bring back the upper-lid show, and make you look refreshed. If you have both problems, tackling only one issue is not the solution. Even though it may sound easier and be less expensive, addressing both concerns through both procedures is often the best way forward.

48 years of age, and looking better than ever.

This gorgeous patient was noticing some laxity in her lower face and neck and wanted to refresh the early changes she was noticing. As part of her Portland Premier Lift 10 months prior to this photo, she underwent:

✅ Preservation deep plane facelift
✅ Deep neck lift with submandibular gland reduction
✅ Fat transfer

At 10 months out, she is looking beautifully refreshed and stunning. Of course her eye makeup is different from the before photo, but no surgery was done on the upper face in this patient.

Tired of looking tired? 👇

Lower eyelid bags are one of the most common cosmetic concerns, but not every under-eye issue requires surgery. When discussing rejuvenation with my patients, I typically see 3 main concerns:

1️⃣: Crepiness

Crepey skin doesn’t automatically mean you need surgery. Chemical peels and lasers can often significantly improve skin quality and texture, making them a great starting point.

2️⃣: Puffiness

Puffy under eyes are a perfect reason to seek out a lower blepharoplasty. I reposition the excess fat lower to create a smoother transition to the lower face using a transconjunctival approach, meaning the incision is made inside the eyelid to avoid visible scarring.

3️⃣: Hollows

True hollows are caused by a lack of volume, creating a sunken appearance, shadows, or dark circles. What appears to be hollowness can sometimes be your skin color, which is not correctable with surgery. In those cases, makeup may be the most effective option for concealing the appearance.

When true volume loss is present, adding volume can help. Filler may be appropriate in some cases, but fat transfer often provides the best long-term result. We often combine treatments such as lasers, fat transfer, and lower blepharoplasty depending on the patient.
